The CCSS, known locally as la Caja, is Costa Rica's universal public health insurer and provider, established in 1941 as a social security system for wage earners and expanded through the 1960s and 1970s to cover rural populations, farmers, and independent workers. Funded primarily through a payroll tax, it delivers comprehensive care with no deductibles or premiums based on age or pre-existing conditions, mandates free treatment for mothers, children, indigenous people, the elderly, and people with disabilities, and by 2024 extended services to more than 95 percent of the country's population.
