A Dutch law, known by its Dutch name Zorgverzekeringswet or by the abbreviation Zvw, that took effect on January 1, 2006 and restructured the Netherlands' health financing system into a single mandatory scheme covering the whole population. The Health Insurance Act folded the country's previously separate statutory and private health insurance arrangements into one scheme administered under private law, requiring everyone who legally lives or works in the Netherlands to buy a policy from a private insurer; insurers, in turn, are required to accept every applicant at the same community-rated premium regardless of pre-existing conditions. Residents pay a flat nominal premium directly to the insurer of their choice plus an income-related contribution deducted through payroll, and because virtually every Dutch health insurer operates as a not-for-profit cooperative, any surplus is returned as lower premiums or added to required reserves rather than distributed as private profit.
