A Malawian government approach to preventing mother-to-child HIV transmission, developed by Malawi's Ministry of Health and put into practice in the third quarter of 2011 in response to the practical failures of an earlier CD4-count-based approach, which had been hampered by Malawi's limited laboratory capacity and by delays that let treatment gaps develop. Under Option B+, every HIV-positive pregnant or breastfeeding woman becomes eligible for free antiretroviral therapy for life, regardless of her CD4 count or clinical stage, replacing a more complex protocol that had frequently started and stopped women on treatment in ways that risked drug resistance. Among the first cohort of 2,949 women who began antiretroviral therapy under the approach in its opening quarter and remained in care, 77 percent, or 2,267 women, were still receiving treatment after 12 months, a retention rate comparable to that of Malawi's general adult HIV treatment program, and the World Health Organization subsequently recommended the Option B+ approach globally.
