CDC WISEWOMAN Program

Chronic Disease Prevention Program

A United States public health program, established after Congress authorized the Centers for Disease Control and Prevention in 1993 to extend cardiovascular screening and services to women through the existing National Breast and Cervical Cancer Early Detection Program. WISEWOMAN, an acronym for Well-Integrated Screening and Evaluation for Women Across the Nation, screens low-income, uninsured and underinsured women aged 40 to 64 for cardiovascular risk factors such as high blood pressure, high cholesterol and diabetes, then connects them to services supporting heart-healthy changes in diet, physical activity, tobacco use and medication adherence. Between 2008 and 2013 alone the program screened nearly 150,000 women, more than 90 percent of whom were found to carry at least one cardiovascular risk factor, and it continues to operate by funding state health departments, tribal organizations and a U.S. territory to deliver the screening and lifestyle-support services in their own communities.

Delivers Practice

Entity-backed identity for the program-kind enum value this public health program already carries, resolved to a health practice by an explicit value-to-entity map (phase 3 bucket conversion, docs\design_entity_backed_browse_buckets_20260928.md). The program-kind fact itself stays on the program unchanged.
