It instituted major reforms to the country's health care system by automatically enrolling all Filipino citizens in the National Health Insurance Program and granting them immediate eligibility for health benefits, dividing members into direct contributors who pay premiums and indirect contributors whose premiums are subsidized by government. The World Health Organization characterized the law as the culmination of several decades of Philippine health sector reform, addressing a fragmented prior system in which out-of-pocket spending had accounted for roughly fifty four percent of the country's total health expenditure before enactment.
