A South African community health worker program introduced in 2012 as one of three components of the National Department of Health's Primary Health Care Re-engineering strategy, intended to improve communities' access to health services and the quality of care they receive. Each Ward-Based Outreach Team, commonly known as a WBOT, is attached to a primary health care facility and made up of a team leader, usually a professional nurse, together with five or more community health workers, each responsible for supporting between 250 and 400 households in home visits covering child, adolescent and women's health promotion, basic preventative care, chronic-disease adherence support, home-based care and early detection of illness. The model traces its conceptual roots to the 1940s, when physicians Sidney and Emily Kark built the first health system in South Africa's Pholela district in KwaZulu-Natal to systematically link health providers directly to the people they served, a lineage South African health researchers explicitly connect to the modern WBOT design.
