China's modern tuberculosis control program, built around the World Health Organization recommended DOTS strategy, which China began piloting in 13 of its then 31 provinces in 1991 with financing from a World Bank loan after decades of earlier, less standardized TB control efforts dating to the 1950s. The program removed diagnosis and treatment fees for patients to reduce financial barriers to care, and DOTS coverage was expanded to all Chinese provinces by 2002, reaching full coverage around 2004. Within three years of expansion, the share of counties implementing DOTS rose from 68 to 100 percent and detection of smear positive tuberculosis cases by the public health system more than doubled, from about 30 to 80 percent of new cases, leading the World Health Organization to describe it as one of the most successful DOTS programs in the world.
