National Health Insurance Scheme (Ghana)

A nationwide health insurance scheme established by the Government of Ghana in 2003, replacing the country's earlier cash-and-carry system in which patients paid for services out of pocket at the point of care. It is financed substantially through a dedicated National Health Insurance Levy added to value-added tax, supplemented by member premiums and a portion of social security contributions, with exemptions for children, the elderly, pregnant women and the very poor. Active membership and renewal rates have fluctuated considerably since launch, and researchers have documented ongoing debate over how equitably enrollment, renewal and benefits reach the poorest Ghanaians in practice. It nonetheless remains one of sub-Saharan Africa's most closely studied national health insurance efforts and has influenced the design of health financing reforms in several neighboring countries.
